SRHT Health and Social Services’ mission is to assist in stabilizing the health and wellbeing of residents by providing wrap around services and medical care to maximize personal goals and objectives.
SRHT HSS started in 1993 by offering Case Management services to Skid Row Housing Trust residents. Since then, we have expanded to offer a holistic range of clinical and social services while working with area partners to continue to affirm the human rights and dignity of our nearly 2,000 residents.
We provide individualized care plans unique to each resident’s needs to successfully maintain permanent supportive housing. Our team operates within a community model that encompasses clinical support, intensive case management, and organizational collaboration. Our goal is to work towards the improvement of our residents’ medical health, mental health, and overall wellbeing.
The security offered by a permanent home is the first step towards safety, stability, and wellness. When a person experiencing homelessness becomes housed, SRHT HSS provides the opportunity for low-barrier access to health care, substance abuse treatment, mental health treatment, and holistic opportunities for self-sufficiency that support residents’ autonomy and stability, and dramatically reduces their reliance on emergency services.
